Sioux Falls, SD (KELO-AM) There was a fatal accident on the job Monday morning in Sioux Falls.
Sioux Falls police say that emergency services were called to Josten Concrete Products Company on South Van Eps Avenue for an industrial accident. They say a 3,200 pound concrete form fell and struck and killed 59-year-old employee, David Lee VanVoorst of Sioux Falls.
