SIOUX FALLS, S.D. (KELO AM) - Sioux Falls police say 59-year-old David Lee VanVoorst was cleaning the bottom of a concrete cylinder Monday morning when it fell from the hoist killing him.
Officer Sam Clemens says their investigation shows a failure of the unit used to hold the cylinder. He says it appears that one of the pins holding the 3,200 pound cylinder failed and it fell from the hoist.
Clemens says VanVoorst was working alone when the accident happened but the noise alerted co-workers who lifted the concrete off him and began c-p-r.
Clemens says police have handed over all their materials to OSHA is now handling the investigation.
